This voluntary program helps families just like you find and use community services that are designed to support families. There is no change for your family to participate.
Pregnancy Care Management
The Pregnancy Care Management program provided formal case management services to eligible women during and after pregnancy and intervention as early in pregnancy as possible to promote healthy pregnancy and positive birth outcomes.
Maternal Outreach Worker (MOW)
The Maternal Outreach Worker (MOW) program is an enhancement of the Maternity Care Coordination and Child Service Coordination programs. Para-professionals offer outreach and support services through home visitation to pregnant women and infants up to one year of age. This community-based program extends support to isolated and alienated women and children who do not typically receive preventive health services.
Project HOPE
Project HOPE is a program that offers interpersonal skills classes during the school year. The target population is at risk youth that are attending West Mart Alternative School in Oak City.
Healthy Start Baby Love Plus Program
designed to address racial disparities in infant mortality. The intent of the program is to improve access to and utilization of prenatal and postpartum services and assure that women receive the necessary services required for a healthy birth outcome.
Post Partum Home Visitation
New Mom and Newborn Assessments
Martin-Tyrrell-Washington District Health Department offers home visits by a registered nurse after hospital discharge for new mothers and babies who are on Medicaid and/or received prenatal or care coordination services from the Health Department.

Communicable Disease Control
Communicable diseases are infectious and can be spread from person to person or from a single source to a group of people. The role of the public health department is to prevent and/or control the spread of communicable diseases by disease surveillance, investigation of reported cases of communicable disease, and application of measures to control the spread of verified communicable diseases.
Some of these control measures are mandated by North Carolina Statutes and others are recommended by the Centers for Disease Prevention and Control. All health care providers and laboratories are required to report to the local health department cases of suspected communicable diseases in persons who have addresses in that jurisdiction. Investigation and testing are confidential, and some services are provided free of charge by the Health Department.

Martin County Family Support
- To provide activities for parents and caretakers which are designed to promote nurturing parenting skills and to increase protective factors in the homes of Martin County residents
- The funding for this program is through a grant from the North Carolina Division of Social Services, Martin County United Way and East Carolina Behavioral Health
Services Provided: Nurturing Parenting Skills Building Classes per year; monthly Child Development workshops, monthly Parents Support Group sessions, Fatherhood Development classes, Baby Care Workshops for first time parents and home visitation.

Juvenile Restitution & Community Service
- To provide youth of Martin County an opportunity to complete court ordered hours towards compensation for victims
- Staff receive referrals, admit the youth into the program, schedule work sites, provide supervision during the work sites and maintain contact with the referring juvenile court
- Funding for this program is made possible by the Martin County Juvenile Crime Prevention Council.

Martin County Teen Court
- To provide a diversionary option for youth between the ages of 11 and 17 that are first time offenders
- Staff receive referrals, admit the youth into the program, schedule teen court sessions, monitor the youth’s progress towards completion of their dispositions and maintain contact with the referring party.
- Funding for this program is made possible by the Martin County Juvenile Crime Prevention Council and the Martin County United Way.

Washington County Adolescent Parenting Program
- To provide intensive case management services to pregnant and parenting teenagers that reside in Washington County
- Primary purpose is to reduce the second or higher birth order pregnancy of these teenagers
- Encourage and support the youth in completion of their high school or high school equivalency as well as post secondary education
- Encourage the participant to utilize appropriate health care for themselves and their child
- Reduce the instances of child abuse and neglect
- Provide opportunities for program participants to prepare for employment
- Work closely with the Community Advisory Council on any programmatic challenges
